The evolution of treatment options for HIV-infected patients has both simplified and complicated the selection of an initial ARV regimen. PI-based regimens have always been associated with the absence of PI resistance mutations and with less NRTI resistance at the time of treatment failure. The rationale for using initial NNRTI-based regimens, on the other hand, has been their efficacy and simplicity. A number of recent head-to-head trials have illustrated the complexities involved in comparing the outcomes associated with these initial regimens and the effect of these regimens on future treatment options.
